Online GLP-1 prescribers issue prescriptions from a chatbot intake and a video call. They do not measure body composition, visceral fat, metabolic rate, VO2 max, or fasting insulin before prescribing. They do not build a muscle preservation protocol. A muscle preservation protocol is a structured combination of protein prescription (calculated from your resting metabolic rate, not estimated from a formula), strength training programming built around your VO2 max-derived training zones, body composition monitoring at every visit, and physician oversight of medication dosing relative to muscle loss. Without this, GLP-1 weight loss often comes 30 to 40% from muscle, which permanently lowers metabolic rate and increases the risk of weight regain.
